 If you would like to register as a patient at Westside Medical Centre, please download the form below and fill in all relevant information. When bringing in your forms to surgery please provide two forms of identity one photo and one other (e.g. Letter with name and address). If you do not have any forms if identity, please do not worry - please come in to surgery and ask to speak with one of the Reception Managers who will be able to assist you.

Temporary Registration

If you are unwell whilst you are away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for up to 3 months. 

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient at your permanent GP. After three months, you will have to re-register as a temporary patient or permanently register with us.

To register as a temporary patient, simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

The Doctors would like to welcome you to the Practice. We have today registered you as a patient. We would like to invite you to attend a New Patient Health Check with the Health Care Assistant so that we can update our records with any relevant history and medication that you may be taking prior to receiving your full medical records from your previous surgery.

Your appointment will take approximately fifteen minutes to enable the Health Care Assistant to take your blood pressure, height, weight and test your urine, and ask about your general family history. When you book in for your appointment, please ask at reception for a bottle for a specimen of your urine for testing at your appointment. This will also allow you an opportunity to discuss any health issues with the Health Care Assistant who will then be able to direct you to the correct clinician.

This practice has a policy stating that if you are 10 minutes or more late attending for an appointment you will be asked to rebook your appointment. Please browse the website for further practice policies.

Please note that if you are on long term medication then we will need a copy of your current medication from your previous practice stating your correct medication and dosage. Without this confirmation we may not be able to issue your repeat medication. Repeat prescription requests must be made 48 working hours prior to being ready for collection.

If you cannot bring a translator with you to the surgery, please speak to the receptionist who will be happy to arrange this for you.  Please note, we are required to give a minimum of 48 hrs notice to arrange a translator.
